If you have been denied access to medical records of your child and a court has not specifically ordered that you are not entitled to the records then you may wish to file a Motion for Rule to Show Cause. IC 16-39-2-9 is the state law which provides for your rights to access your child’s medical records.
06D02-0703-DR-0076 RTSC 10-08-09 is a sample of a Motion for Rule to Show Cause for failure to provide mental health records.
